It's time for your AHA! moment.
By Phone: Call (816) 744-8019 and request a ‘Blood Draw’ appointment on the Nurse Only schedule, and be sure to mention it is for an AHA Screening
Online / App:
Login to My Premise Health online or in the app with your username and password (If you are not already registered, click here for registration instructions, or contact Member Services if you need assistance)
Select ‘Schedule an Appointment’
Select 'Biometrics' from the list as the reason for your visit, and then type ‘AHA Screening’ under ‘What is the most important thing you want addressed during this visit’
On the day of your AHA Screening appointment please remember:
Be sure to fast for 10 hours, no food or drink except water for 10 hours before your AHA appointment time, and
Drink plenty of water to be sure you are properly hydrated for the blood draw.
Getting your results:
Within 3-5 days following your appointment, you will receive an email notifying you that your lab results are ready to review in a detailed AHA summary booklet available in your patient portal. (Be sure your settings for email notifications in the portal or app are not turned off!) Rest assured, this is the same booklet you’ve received by mail in prior years. To be conscious of the environment, booklets are not being printed. However, the electronic version will be in a print-friendly format for you if you’d like a paper copy. If you have not received an email within 5 days, your lab results can be found on My Premise Health by clicking 'Test Results’.
Lastly, if you would like to review your results with a provider, you may schedule an AHA Follow Up visit at the Complete Health & Wellness Center.
