How to Register

Sign up for My Premise Health today to get started.

Registration is required prior to being able to make an appointment. Each eligible family member must be registered separately.

Note: You cannot register until your insurance is effective.

Self-validate your My Premise Health account directly, online or on the mobile app:

  • Go to mypremisehealth.com, or download and open the My Premise Health mobile app.

  • Click the “Sign up now button”

  • Enter the following information: your full name, date of birth, and the last four digits of your social security number (SSN).

  • Then, hit “Submit”.

Note: New account activation is required for all patients prior to being able to make an appointment. If you have newly enrolled in the District’s medical insurance plan, you will be unable to activate your account until your insurance is effective.

Each eligible family member will need to be registered separately. To allow viewing by a parent or guardian, the parent or guardian can select ‘Request Minor Chart Access’ from the ‘Menu’ in their portal. For assistance, contact Member Services.
