Appointment Scheduling
Appointments are required. Please schedule an appointment prior to arriving at the Complete Health & Wellness Center through one of the following methods:

To schedule an appointment, sign into My Premise Health online or in the app and click the 'Schedule an Appointment' icon.
To send a secure message to health center staff, sign in to My Premise Health online or in the app and click the 'Send a Message' icon. To view sent and received messages, select 'Messages' from the Menu list.

Hours are subject to change. Closed major holidays.*
Appointments are required. Please do not walk in.
Monday: 7 AM - 11 AM, 12 PM - 6 PM
Tuesday: 7 AM - 6 PM
Wednesday: 7 AM - 11 AM, 12 PM - 5:30 PM
Thursday: 7 AM - 5:30 PM
Friday: 7 AM - 12 PM
Saturday: 8 AM - 11 AM
Sunday: Closed
*Will be closed New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, the day after Thanksgiving and Christmas Day. When a holiday falls on a non-workday the holiday will be observed on Monday (if the holiday falls on Sunday) or Friday (if the holiday falls on Saturday).
For Outside Lab Orders:
All outside lab orders should be faxed to the Complete Health & Wellness Center prior to your appointment to minimize your wait time.

Scheduling Details
Appointments are Required: Please schedule an appointment prior to arriving at the Complete Health & Wellness Center, please do not walk-in. This helps ensure you and your fellow staff members have the shortest wait times and best experience possible.
Scheduling for First Time: If you are you scheduling for the first time, you must first be registered with My Premise Health. Click here for instructions on how to register. (You cannot register until your insurance is effective.)
Urgent Needs: If you have an urgent need and are unable to find a same day or next day appointment, please either call Member Services or send a secure message through My Premise Health using the 'Send a Message' feature to see if they can accommodate you.
Wait List Feature: After you schedule an appointment through the My Premise Health portal and would prefer to be seen sooner than your scheduled appointment time, you can add your name to a wait list to get notified if an earlier appointment becomes available. When you confirm your appointment during the scheduling process, it will state ‘Wait List Available’ and have the option to select ‘Get on Wait List’.
Virtual Visits Only Available for Certain Appointment Types: Virtual Visits by phone or video are only available for certain appointment types such as cold, flu, follow ups, lab reviews, medical questions, allergies, and prescription refills. Click here for more information on Virtual Visits.
Cancellations: If you are unable to make your appointment time, please call ahead to cancel or reschedule as early as possible, out of courtesy for other patients and the Complete Health & Wellness Center staff. You can modify an upcoming appointment in the My Premise Health portal by locating your ‘Upcoming Visits’, click on the specific appointment, and then select the option to either ‘Reschedule appointment’ or ‘Cancel appointment’. You can also call Member Services at (816) 744-8019 to cancel or reschedule appointments.
First Visit Instructions
Prior to your first visit, you will be asked to complete new patient materials.
To save time, complete the paperwork before you arrive by logging in to mypremisehealth.com or through the app. Select eCheck-in to complete all required forms and verify your health information. Click here to learn how to eCheck-in.
If you are unable to provide your information via mypremisehealth.com, please plan to arrive 10 to 15 minutes prior to your appointment time to complete all necessary paperwork.
Bring your Driver’s License and Blue KC Member Insurance Card with you to your visit. Your insurance card is also available electronically in the MyBlueKC mobile app.
