With the secure My Premise Health patient portal and mobile app, managing your health is easier than ever.
Whether you’re on the go or at home, you can easily:
Conveniently schedule same-day or next-day appointments.
View lab results as they process.
Manage your medications and request prescription refills.
Meet with a provider virtually via phone or video.
Securely message your provider.
Fill out forms and check-in ahead of time.
Quick Tips:
To schedule an appointment, sign into My Premise Health online or in the app and click the 'Schedule an Appointment' icon. (Additional details on appointment scheduling for in-person and virtual visits are available in the documents below.)
To join the wait list, if you schedule an appointment through the My Premise Health but would prefer to be seen sooner than the available appointment time you selected, you can add your name to a wait list to get notified if an earlier appointment becomes available. When you confirm your appointment during the scheduling process, it will state ‘Wait List Available’ and have the option to select ‘Get on Wait List’.

To send a secure message to health center staff, sign in to My Premise Health online or in the app and click the 'Send a Message' icon. To view sent and received messages, select 'Messages' from the Menu list.
Note: Secure messaging now available through My Premise Health is replacing the direct CH&WC email and that inbox will no longer be monitored. If you have a medical question or need to contact CH&WC providers (for things such as medication refills, non-urgent medical questions, test result questions, visit follow up questions, etc.), please use the secure messaging in My Premise Health.
To view lab results, sign in to My Premise Health online or in the app and click the 'Test Results' icon.
